Registering Power of Attorney

After downloading and printing your generated documents from the Accounts & Assets section, kindly proceed with the following steps to ensure their registration and legal validity.

  1. Please conduct a comprehensive review of the forms to ensure all fields have been accurately completed.
  2. As the "Donor," you are required to sign and have witnessed Sections 9 and 15. Additionally, for a "Health & Welfare" LPA, please ensure to sign Section 5.
  3. The "Certificate Provider" must sign Section 10.
  4. All "Attorneys" will need to sign and have witnessed section 11.
  5. In Section 14, please specify how you would like to pay the £82 application fee.
  6. Once the entire form is complete, you can post your application to the address listed below.

Important Information

It is crucial for the Donor to sign all sections of the document initially, followed by the Certificate Provider, and finally, the nominated attorneys. This sequential order ensures clarity and validity in the execution of the document.

Witnesses

In the case of the Donor, the witness must not be an attorney registered on the document. Similarly with the registered attorney, the witness cannot be the Donor. All witnesses must be aged 18 or over.
