Creating a Care Plan

From a Mobile Phone

1. Click the menu icon in the top right corner.
2. From the menu, select ecg My Advance Care Plan
3. From the Care Plan Overview page, click the edit_square icon in the top right corner

From a Laptop or Tablet

1. From the left side navigation, select ecg My Advance Care Plan
2. From the Care Plan Overview page, click the edit_square icon in the top right corner

Be sure to complete all the sections and then click the save button in the top right hand corner. You can always come back and edit your care plan at a later stage.
